Unvieling the Wearable Innovation: AIKON Health's Heart Failure Monitoring Solution

Frequent readmission due to heart failure has a significant impact on the patients, their families and on the health system. Following initial treatment, individuals may face the risk of returning to the hospital often due to inaccurately titrated medication or non adherance to the guideline recommended medication. Effective post-discharge care is essential in minimizing the likelihood of readmission, medication management and lifestyle adjustments.

We aim to reduce readmission of heart failure patients with a proprietary device enabled solution to assist Cardiologists and Cardiology nurses in effective management of the condition. The wearable device solution will measure key vital sign parameters such as the thoracic fluid overload, cardiac output and stroke volume that are relevant for monitoring HF patients. Digital Biomarkers, characteristic of heart failure, can provide indicators of early deterioration and can help Clinicians in identifying patients for intervention at the right time.
